We are collecting all information contained in this registration form on behalf of the Australian Government Department of Health and Ageing. The Commonwealth Privacy Act 1988 requires that you provide your consent to the inclusion of your personal information, such as name, employer and email, in the list of delegates for this Conference. This list will be made available to all other Conference delegates and Conference supporters/sponsors for the purpose of distributing related material. Members of the Casemix Conference 2008 Steering Committee will also receive copies of the list of delegates to enable the Committee to forward Conference information and materials to delegates if required. This list will also be provided to the organisers of the next Casemix Conference scheduled for 2010. This list will not be provided by the Casemix Conference 2008 Steering Committee to any other third party.
